This information is for New Patients

What is the cost?

New patients are scheduled for either an hour ($300) or an hour and a half ($375).

If a person does not have lab or clinician reports to review, then it’s an hour consultation.

If there are reports to review then it’s an hour and a half.

Return visits can be a half hour ($125), 45 minutes ($175) and one hour (4200)

Neither the visit or supplements are covered by insurance providers.

For lab testing, which is primarily blood labs, there is the option of paying us at a discounted price or we can provide a letter requesting labs which lists the labs and the diagnostic codes for insurance (letter $20).  There is no guarantee that your insurance company will cover these labs but almost always they do.

If for any reason your insurance company forwards insurance claim forms, we reserve the right to not comply.

For HSA cards, all costs related to consultations, treatments and labs are covered.  For supplements, HSA may or may not work.

If you schedule an initial hour appointment because you don’t have reports to review and at the end of this consult Dr. Haskell recommends getting labs, the second appointment will be a half hour ($125) to review these results.

If you schedule the initial hour and a half appointment because you have reports to review, very often there are some labs that have not been done and need to be ordered.  Once they are don’t a second half hour appointment will be scheduled to review them.

Typically, future appointments are half an hour and scheduled a month out.

To schedule, please call 801.875.9292 or 801.209.3363 and speak with Espe.  You can also text the 3363 number.
